Withdrawal card Dear Sisters and Brothers Please be sure to request a withdrawal card when laid off or terminating your employment. To obtain a withdrawal card a member's dues must be paid through the month in which the card is issued. The cost of the withdrawal card is fifty ($0,50) cents. Please feel free to call this office with any questions concerning this matter.
